The person completing this form will become the Super-Administrator for your organization’s homepage. This account will control access to PHI, manage referrals, and oversee user permissions across sites.

If you are not the intended Super-Admin, please do not complete this registration. Instead, refer this page to the appropriate administrator for your organization.

• One account per location: Each site under your organization must complete its own registration with unique login credentials and a distinct display name. To avoid confusion when selecting appointments, include the street name after the organization title (e.g., MATTERS - Goodell, MATTERS - Delaware). This ensures clarity across multiple entries.
• Linking multiple sites: If one Super-Admin manages several locations, register each with the same email, username, and password. After registering the first location, you’ll receive a confirmation email with your Organization Group Code. Enter this code when registering each additional site to link them under the same administrative group. This allows centralized access and oversight across all linked sites.
• Peer services must register separately: If your organization provides Peer Services independent from treatment, they require a distinct account. During registration, select 'Peer' under Service Category, and reflect the distinction in the title (e.g., MATTERS - Goodell for treatment, MATTERS - Goodell Peer for peer services).
• After registration: You can add more users to each site as needed through the Super-Admin portal.
